How to Manage Cervical Incompetence by Application of a Cervical Cerclage Suture in the Pregnant Mare
نویسنده
چکیده
Cervical incompetence in the mare is widely recognized as a major cause of pregnancy failure. Failure to attain adequate closure during pregnancy is a key predisposing factor to ascending placentitis. In humans, cervical insufficiency is of major concern and can result in preterm birth and lateterm abortions, similar to observations in mares. Emerging clinical and laboratory-based evidence suggests that focusing on the uterine cervix may provide significant elements of diagnostic value to identify patients at risk for preterm delivery, offering guidelines to implement preventive intervention. A variety of techniques have been used in human medicine to address cervical incompetence, including medical therapy with progesterone, cervical cerclage, and the placement of a cervical pessary, some of which may be applicable in the mare. A recent study in human medicine compared vaginal progesterone versus cerclage, with the use of placebo/no cerclage as the control treatment. The authors concluded that progesterone and cervical cerclage are equally effective for the prevention of preterm birth and adverse perinatal outcomes in patients with a short cervix and history of preterm birth. A common treatment strategy used on mares that have previously lost a foal because of placentitis or that have anatomical abnormalities in the caudal reproductive tract making them at risk of ascending placentitis is to administer an antimicrobial, a nonsteroidal anti-inflammatory drug and altrenogest daily until delivery of the foal. The shortcoming of this treatment is that mares may still have a preterm delivery/fetal loss with histopathological evidence of ascending placentitis despite treatment. This report describes a technique for cervical cerclage applied to a population of mares with known cervical incompetence.

ABSTRACT Background: To compare pregnancy outcomes in women at risk for cervical incompetence after elective versus ultrasound- indicated cerclage. Methods: In a prospective randomized tlial, singleton pregnancies with at least one previous spontaneous midtrimester loss or early preterm delivery (between 15 and 32 weeks' gestation) who met clinical crite1ia for the diagnosis of cervical incomp...

BACKGROUND Cervical incompetence is one of the causes of preterm birth and mid-trimester pregnancy loss. Cervical cerclage is a surgical procedure to treat cervical incompetence. Cervical cerclage reduces the incidence of preterm birth in women at risk of recurrent preterm birth, without a statistically significant reduction in perinatal mortality or neonatal morbidity. Multifilament/braided su...

Cervical incompetence is often associated with mid trimester pregnancy losses and preterm labour. cervical cerclage to prevent miscarriage and preterm labour is practiced world wide. The objective of this study was to document the feto-maternal outcome following cervical cerclage for cervical incompetence. The study reviewed all case files of pregnant women who had cervical cerclage for cervica...
متن کاملCervical cerclage for prevention of preterm birth: A commentary
Cervical cerclage remains a proven method for the prevention of preterm birth caused by a ‘weak cervix’. However, there is controversy as regards the precise diagnosis and definition of ‘cervical incompetence’, the actual surgical technique, choice of suture material and number of sutures and ties to be made during the procedure. There is now emerging evidence that other alternatives to cervica...

BACKGROUND Cervical incompetence is one of the main contributors to repeated pregnancy loss, accounting for approximately 25% of the cases. Typically it results in progressive cervical dilatation, leading to a painless second- or early-third-trimester abortion. OBJECTIVES The main objective of the study was to explore the benefit from cervical cerclage in pregnant women with anatomical cervic...
